Person-Centered Planning: Creating a 'Picture of a Life' for Foster Youth with Intellectual and Developmental Disabilities Transitioning to Adulthood

Person-centered planning (PCP) is well regarded as a positive individualized approach for individuals with I/DD to plan for transitions and a desired future based on personal preferences and abilities. However, there is minimal research to substantiate the approach as an evidence-based practice. The Sonoran UCEDD and Arizona Division of Developmental Disabilities are implementing a program for foster youth with I/DD and evaluating the efficacy of PCP, facilitator behaviors, and cross-systems collaboration to improve outcomes of transition to adulthood.
