Product Description:

The journey from adolescence to adulthood often crosses an abyss in care. Neither pediatric nor adult medicine practices are structured to truly meet the unique needs of this emerging adult cohort. They also have not constructed the connections needed to ensure continuity of care for young adult patients. This gap is problematic in several ways for both the individual and society, especially with regard to the estimated 5 million youth with special health needs (YSHN) making this passage.
