HEALTHCARE INTERVENTIONS FOR INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES: EVIDENCE OF IMPROVED QUALITY AND COST EFFECTIVENESS

This present project sought to evaluate a specialized medical home for individuals with IDD - specifically the DD Health Home (HH). The DD HH was developed to provide comprehensive primary health care services in normalized, community based office settings that go beyond typical primary care to incorporate care coordination elements of specialty care.
Through various affiliated practices, the DD HH has collected extensive patient data from more than 800 IDD patients in New Jersey and New York. This project aggregated and analyzed outcomes from this database to evaluate the overall efficacy of the DD HH model by analyzing data on IDD patients including: 1. presentation characteristics and co-morbidity, 2.functinal health status, 3.metal health status, 4. Satisfaction with care and 5.health care utilization and costs. Findings indicate that a model such as medical health home that focuses specialized care on IDD patients will have a positive outcome and accomplish the triple aim.
