Product Description:

The concept of transitions of care (TOC) is gaining increased attention but lacks clarity and holds incommensurable meanings. A concept analysis was conducted guided by the methodology of Walker and Avant. The analysis identified four defining attributes, along with antecedents and consequences of TOC. This analysis provides conceptual clarity that can be used to guide the development of a middle-range theory, ultimately to guide clinical practice and inform health policy.
