Transportation: A Vehicle for health equity in rural communities.

Transportation is a driver of health equity; it compounds effects of other social determinants of health and is a lower-cost intervention. Due to restricted transportation options, rural communities frequently experience limited access to medical care and services, particularly those related to substance abuse.
This session presents a rural health intervention for high-risk pregnant women and describe how transportation, provided as an element of care coordination, has increased social support and improved health care services. Additionally, in this session benefits of enhanced care coordinator-client-physician relationships resulting from incorporation of transportation into flexible client support services are discussed.
