Product Description:

The Family Care Enhancement Project at MMI places Parent Resource Coordinators in clinical locations across the state of Nebraska. This model allows PRCs to be available onsite for families to assist in care coordination as well as assist doctors in providing family-centered care. This model also offers the enhanced benefits of providing both peer mentorship and care coordination--activities that benefit both the clinic and the families they serve.
