Transition to Adulthood for Young Adults with Developmental Disabilities: A Timeline for Parents, provides a one-page overview of considerations at different points throughout transition, national resources, and information about services in NJ. To ensure this resource reflects the needs of families, the UCEDD/LEND partnered with a diverse group of parent leaders. This presentation focuses on the parent-professional partnership for identifying changes to maximize accessibility and applicability.
