Product Description:

Oklahoma UCEDD/CLL staff and Oklahoma Family Network collaborated to create a fillable Charting the LifeCourse Portfolio. The Portfolio is a combination of the trajectory, the one page, and the integrated supports star products into on comprehensive form. This was drafted and piloted for the Deaf-Bind project at the University of Oklahoma Norman Campus during Spring 2020. This form is available virtually.
