This is a brochure for organizational purposes, free for PR families who request it and who have a child with special health care needs. Parents can use this guide to keep track of important information about the health care and educational needs of their child with special health care needs (SHCN). The brochure is available in both Spanish and English and is customized by our F2F staff to suit the unique needs of each child.
