Product Description:

Case managers are lynchpins in the Home and Community Based Services (HCBS) system. This poster presentation outlines the process of working with a group of rural Wyoming case managers to create an online person-centered planning training for fellow Wyoming case managers. The training development was a collaboration between the Wyoming Department of Health, a Case Manager Advisory Board, and the Wyoming Institute for Disabilities (WIND).
