Product Description:

The Home and Community-Based Services (HCBS) and Wyoming Institute for Disabilities (WIND) at the University of Wyoming worked with a team of case managers to develop a Wyoming case manager training. This training is intended to build case manager capacity and increase knowledge and skills to support Wyoming Department of Health's commitment to quality services for Wyoming families by focusing on the skills for co-developing person-centered plans of care as defined by the HCBS Settings rule.
