Product Description:

The MFP Transition Plan describes the process for implementation of pre and post-transition phases for individuals who are deemed eligible for Medicaid and fall into the two target populations of 65 years and over and/or has a physical disability. This process is governed by Person Centered Planning so that the individuals have the voice and choice to transition from the institution and integrate into the community; to live in the home of their choice.
