Product Description:

Center for Medicaid Services requires every funded Money Follows the Person (MFP) program to develop an Operational Protocol. In addition to how the project will be implemented, a Transition Plan, Food Security Plan, and Housing Plan are part of the entire OP document. Once the OP is approved by CMS, the project will be implemented. All plans (Housing, Transition, and Food Security) have gone through public comment. The stakeholder Advisory will review it during July's quarterly meeting.
