The Kansas DDS Resource Guide is a quick desk reference guide for Disability Determination Service (DDS) adjudicators to identify state or county disability programs in the state of Kansas that can refer or provide medical and other evidence for SSI childhood applicants or eligibility redeterminations. The reference guide is available in both a paper and electronic format using a database template with Microsoft ACCESS software. The electronic format allows readers to connect to multiple references to same topic within the document. It also lists relevant web sites that provide additional information. The Resource Guide will cover the full range of programs for children from birth through age 21 (SSA begins using adult eligibility criteria at age 18) and is intended to describe agencies or programs that may have relevant evidence for DDS disability adjudicators about the nature and severity of an applicant’s impairment(s). The content for each entry includes the following information: -Name and short overview of program or organization -Eligibility criteria -Services -Location of records -Available assessments [programs should have that may help DDS evaluate the severity of the child’s physical, mental or cognitive impairments] -Level of contact [county or state] -Contact information: by office name, not specific individuals -Internet information and pertinent URLS
