Product Description:

The IOD and the Area Agency for Developmental Services of Greater Nashua are pleased to present this Transition Manual to parents, young people with disabilities and the individuals who provide services and supports to them. This manual was developed through a dedicated effort to support young people with developmental disabilities to develop the skills they need to become self-determined participants in adult life. It addresses vital information regarding public benefits, employment, special education, and other issues that are critical to a successful transition to adult life. This valuable resource also explores Area Agency eligibility and services and includes planning and assessment formats for families and individuals to use.
