After participation in this module, students will: 1) Define transition 2) Discuss barriers to effective transition and strategies for overcoming them 3) List supports available to the community to facilitate transition 4) Outline public policy concerning the transition process 5) Discuss the role of healthcare professionals in facilitating self-advocacy 6) Describe how individuals with disabilities create and use a circle of supports 7) Outline an approach (within their discipline) to assessing capacity and supporting optimal independence in youth with disabilities.
