Otitis Media with Effusion and Child Development: Rethinking Management

Results of a recent large, prospective study challenge the long-held assumption that developmental delays in young children, especially in the domains of speech and language, can be attributed solely to persistent middle-ear effusion (MEE) and possible accompanying conductive hearing loss in the first few years of life. A consortium of major professional organizations has re-issued a practice guideline that calls for a conservative and prudent, but not passive, approach to the management of persistent MEE in infants and young children. This paper reviews the rationale for the recommendations in the guideline and provide an algorithm for management. Watchful waiting regarding persistent MEE should not be misconstrued as watchful waiting regarding developmental delays, even in children with persistent MEE.
