Child Health Notes

Child Health Notes are a series of newsletters providing primary health care providers with current information on the early identification and management of special health and developmental concerns. A goal of the Child Health Notes is to facilitate partnerships between primary health care providers, families and community early intervention providers and other health promotors. This is a tool to support Medical Homes for children with or at-risk-for special health needs. The Child Health Notes are formatted such that a local health department or medical home team customizes the resource information to promote awareness of local early intervention resources. The newsletters are designed to be used and distributed by community-based health departments, local health programs, health care plans, and the like.
