Product Description:

The focus of this report is the Support Coordinator Study that was coordinated through Arizona's Division of Developmental Disabilities. It builds on the results of the first Aging Caregiver Study. The support coordinator survey (SCS) was developed based on the findings of the Aging Caregiver Study and addresses late‐life transitions for caregivers and adults with developmental disabilities (DD). The goals of the SCS were to:<br> * Understand support coordinators' perceptions about aging and future‐care planning<br> * Identify additional training needs for support coordinators as they assist families in making future‐care plans<br> * Identify potential policies that will promote effective transitional care planning
