Product Description:

This panel presentation will discuss various aspects of what youth with disabilities, their families and the professionals who support them should consider in preparation for transitioning out of high school to adult living. Panelists will provide a general overview of person-centered planning as a tool for transition planning, post-secondary opportunities and employment, and the transition to adult health care. Panelists will also share information on specific programs, services and resources available in Arizona.
