Product Description:

Family-Centered Transition Planning combines transition training for families with person-centered planning and career exploration to assist young adults to work towards a personally meaningful vision of adult life. This presentation presents the employment, post-secondary, and community participation outcomes of students who graduated from high school since receiving FCTP. Results arecompared with national transition data. Personal stories of the remarkable progress many students have achieved are also discussed.
