Product Description:
At the Center for Disabilities and Development, an evaluation that is authorized and paid for by a child's intermediate education agency. Goal is to provide information to the local education team, including the parents. Diagnoses include ADD, ADHD, dyslexia, and other specific learning disabilities, behavior disorders, cerebral palsy, Down syndrome, mental retardation. Evaluations completed at Center, by conference call, Iowa Communications Network. Parents are contacts prior to an evaluation to provide information and give permission to contact the local education team. Results of other evaluations, current IEP or 504 plan and completed "School report form are gathered prior to the appointment. Evaluation can be designed to answer specific questions of parents and local education team.
