Product Description:

The website for the Adolescent Health Transition Project was first published in late 1999 and is updated continually. The website provides information about health care transition and other transitions for children and youth with SHCN and disabilities and their families and those that work with them. Though specific to WA State, the site has information and downloadable materials that can be used anywhere. Materials posted on the site include the transition timelines for children and youth with disabilities, the Washington State Adolescent Transition Resource Notebook, the Health History Summary, the Adolescent Health Autonomy Checklist.
