Product Description:

This tool was originally developed by Arizona Dept. of Economic Security but was revised by IHD and put into use on a secure technology platform (REDcap) to determine level of care needs corresponding to funding levels provided by DES for individuals with a disability. This revised version was used by an IHD team who first conducted file reviews and recorded evidence of level of care. This result was then verified by a nurse through a site visit and observation of the individual.
