Product Description:

People with DD may have less access to health services and their needs could be unsatisfied. Their transition to adulthood could be difficult due to deterioration in medical services and the impact of disability decreases their satisfaction with life (complex challenges). Alternatives includes identifying main elements for the transition of health services and a person-centered plan. Factors that affect the health transition and competencies to be developed to facilitate this process.
