Product Description:

This is a tool for organizational purposes, free for PR families who request it and who have a child with special health care needs. Parents can use this route to keep track of important information about the health care and educational needs of their child with special health care needs (SHCN) and the steps for receiving services. The route is customized by our F2F staff to help parent tracking to services.
