This fact sheet is a protocol for transition services. It has information, steps to follow and pre-school setting for children from 3-5 years. F2F guide parents through a family centered model to monitor the health and well-being of their children. The F2F will also offer guide to identify a pre-school setting and made a smooth transition to their children first experiences to academic services.
