Product Description:

The 311 Call Option 2 Flyer was created for dissemination to community members to access and receive vaccination services for individuals with disabilities and their families. The 311 Call Center service providers are able to arrange for transportation services for individuals with disabilities to be transported to the vaccination sites, coordinate homebound vaccination services, and make an appointment for individuals with disabilities to get vaccinated at the vaccination sites.
