Product Description:

There are key issues about the transition from pediatric to adult care. Every transition requires a process of adaptation to a new scenario and/or school environment. The service center and the family needs to communicate to facilitate the process of adaptation to a new environment and post school routine". Changes in life includes feelings such as fear, doubt and uncertainty. It's important to recognize and validate the person feelings and pay attention to their needs.
