Product Description:

The Transition-In-Action Clinic generates recommended "next steps" to successful transitions for young adults with a disability and/or significant chronic health care needs. Together with the young adult's family, the multidisciplinary clinic team creates a comprehensive plan for transition that is focused, coordinated and action-oriented, all at no cost to the person.
