Product Description:

This is a tool for organizational purposes, free for PR families who request it and who have a child with special health care needs. Parents can use this criteria to find out if they qualify, as parents of their child with special health care needs (SHCN), to the F2F services. The criteria is available in Spanish and is customized by our F2F staff to suit the unique needs of the parents in Puerto Rico.
