Product Description:

Planning for the transition from early intervention services (Part C) to preschool services (Part B/619) is a very important step in the life of a child and family. For families of children with disabilities transitions from one program to another can be a highly stressful experience. For some families the transition at age three is only one of many transitions that have occurred in the life of their child. For other families this may be the first significant transition that the child and family has experienced. Successful planning is critical not only to families but also to service providers and the continuity of services. This packet has been developed as a resource to enable transition teams to increase their understanding of the issues related to transition.
