Product Description:

This guide will help families develop a life plan and a letter of intent for their family member who has a disability. These can be shared with others to provide crucial insight into the dreams, aspirations, and support needs of the individual. The guide is a do-it-yourself tool that may be completed by the family alone or with guidance from a service provider, and provides a structure for developing a future plan for the individual. The future plan is not a static document, and should be reviewed annually to ensure that information is accurate and current.
