Product Description:

Life Planning for a Person with a Disability will help families to develop a life plan and a letter of intent for their family member who has a disability. These can be shared with others to provide crucial insight into the dreams, aspirations, and support needs of the individual. While not a legal document, the letter of intent communicates important information about the person and can accompany a will, trust, or other legal documents. Developing a life plan or letter of intent sounds simple, however, it is easy to overlook critical details. The Future Planning Guide provides individuals with disabilities and their families the tools needed to create a plan for the future.
