Product Description:

A resource on transition for schools, communities, agencies, families, students, health care providers and others in Washington State. It is organized in loose-leaf format by sections based on content; each section contains individual documents that may be copied and used as handouts. The documents are a sampling of transition materials and resources and are almost all created for families. Communities may add their own resource documents to the notebook.
