This panel presentation will discuss various aspects of what youth with disabilities, their families and the professionals who support them should consider in preparation for transitioning out of high school to adult living. Panelists will provide a general overview of person-centered planning as a tool for transition planning, post-secondary opportunities and employment, and the transition to adult healthcare. Panelists will also share information on specific programs, services and resources available in Arizona. Objectives: *Understand the issues involved with transitioning from childhood to adult living for youth with disabilities *Knowledge on how to tap into resources to help youth transition to adulthood. *Appreciate that all people with disabilities have unique talents and strengths as well as challenges in transitioning to adulthood.
