Product Description:

This panel presentation discussed various aspects of what youth with disabilities, their families and the professionals who support them should consider in preparation for transitioning out of high school to adult living. The interdisciplinary panel of professionals and parents provided a general overview of transition with special attention to transition planning, post-secondary opportunities and employment, and the transition to adult healthcare through a case study format. Panelists also shared information on specific programs, services and resources available in Arizona and nationally.
