Survey: Leadership Project: AltaMed Body Works Evaluation Tool

Product Description:

This survey was developed for a leadership project and includes a set of questions to assess training needs and program outcomes in the CHLA AltaMed Body Works healthy eating program. The program is offered to parents and their children upon referral by the child's pediatrician. Children referred to the program have been diagnosed as overweight or obese based on a BMI measurement. The questions were presented to parents pre- and post-training using a printed questionnaire.The pre-training questions measured parents knowledge, attitudes and behaviors related to healthy eating and nutrition. This information was used to tailor the lesson plans over the seven-week Body Works program. Upon completion of the program, participants were again asked to complete the same set of questions . The questions were pilot tested in the Body Works program and a modified version will be used in future Body Works sessions.
