Product Description:

Poster presentation focusing on the background and results of 5 year evaluation of the PfH waiver program. In 2007 the Missouri Association of County Developmental Disabilities Services studied Medicaid waivers with the goal of learning about innovative HCBS implementation. This study's recommendations to the Missouri Division of DD led to the creation of the Partnership for Hope Waiver. PfH serves as a model for providing needed community services and supports for individuals with DD and their families through a unique Federal, State, and County partnership.
